Important Notice Regarding Covid-19
At Pathways to Care we are in the business of supporting and caring for people.
In terms of our support coordination, this often means we travel to people’s homes, workplaces, hospitals or residential facilities. We have had time to reflect on how we might make our services as safe as possible in light of the outbreak of Corona virus, is it safe to continue to visit multiple people across the day? The answer is, we don’t know.
The only thing we do know, is that we could not bear the possibility that we might pass this illness onto any of you in the course of our work. Currently there is no safer option then to limit the amount of face to face appointments that we do. We will continue to service each of you via phone, email or skype. This may seem like a big decision to make, but it is the only sure thing we can do to limit our potential impact on those who are vulnerable to the disease. Your support coordinator is available on their mobile phone and email and can help to coach you through any of the readily available options for connecting remotely. This is only a temporary precaution, and our staff feel just as conflicted about not being able to see you, as you are that you can’t see them.
If we work together to make this temporary arrangement work, we can do our part to limit the spread of this illness. Intimate Local Knowledge
We have extensive knowledge of the NDIS Registered Providers, unregistered providers, community organisations and mainstream services in your local area that can provide the support you need.
We’ll work with you to help you live an independent life and support you to manage your NDIS plan. To do this, we aim to increase your knowledge of service providers, service agreements and help you resolve any issues that may arise.
Connect to Community Services
We’ll link you with local organisations, keeping you connected to and supported in your community. These include school, healthcare providers, housing and transport services.
Prepare for Planning Meetings
We’ll help you prepare for planning meetings with your Local Area Coordinator or the NDIA. This includes providing your Planner with an update about your goals and the progress you have made throughout the duration of your plan.
We provide Intensive Support Coordination services for clients with complex needs, to help you manage and resolve conflicts and overcome challenging situations.
